Registration of documents for providing the disabled with specialized movement aids
On the portal you can get the service in several ways:
Using EDS:

3. Fill all sections of e-request
4. Sign a request with digital signature
5. In personal account ("History of obtained services") familiarize with notification on your request processing status, which will be sent within the stated period.
Using one-time password:Using one-time password:

4. Fill in a request and sign it using one-time password (SMS will be sent to a phone number indicated in a personal account, registration in citizens` mobile database is required)
5. In personal account ("History of obtained services") familiarize with notification on application processing, which will be sent within the indicated period.
Documents required for submission to "Government for Citizens" Public Corporation":
- An application.
- A document certifying his identity, or an electronic document from the digital documents service (for identification).
- A power of attorney from persons with disabilities, which does not require notarization, when applying to a person who has received a power of attorney from persons with disabilities.
When applying for prosthetic and orthopedic care for persons with disabilities from work injury or occupational disease, additional information is requested:
- about an industrial accident that led to disability;
- on the termination of the activity of an employer – an individual entrepreneur or the liquidation of a legal entity, through whose fault an occupational injury or occupational disease has been received.
Information about identity documents confirming registration at a permanent place of residence, about disability, about the developed IPAR measures, about an industrial accident that led to disability, about the termination of the employer's activities – an individual entrepreneur or the liquidation of a legal entity, through whose fault an occupational injury or occupational disease has been received, contained in the relevant state information systems, city administrations, employment departments, State Corporations receive in the form of electronic documents, EDS of authorized officials.
Participants of the Second World War, persons with disabilities of the Second World War and persons equated to persons with disabilities of the Second World War, attach to the application a certificate of a participant of the Second World War, a person with a disability of the Second World War or a person equated to a person with a disability of the Second World War.
Documents required for submission to service provider:
- An application.

Notification of the registration of documents for the provision of special means of transportation to persons with disabilities or a reasoned refusal to provide a public service.
The State Corporation informs the applicant about the decision by sending an SMS notification to the applicant's subscriber device.
When contacting through the web portal, the result of the provision of public services is sent to the applicant's "personal account" in the form of an electronic document signed with an electronic digital signature (hereinafter - EDS) of an authorized person.
1) establishing the unreliability of the documents submitted by the applicant for obtaining a public service and (or) the data (information) contained therein;
2) non-compliance of the applicant and (or) the submitted materials, objects, data and information necessary for the provision of public services with the requirements established by the Rules;
3) the absence of the consent of the service recipient, provided in accordance with Article 8 of the Law of the Republic of Kazakhstan "On Personal Data and their protection", for access to restricted personal data required for the provision of public services.

General requirements for the appeal
The appeal submitted in hard copy or electronic form shall indicate:
- Full name, IIN, postal address of a natural person, BIN of a legal entity;
- name of administrative body, official to whom the appeal is submitted;
- the essence of an appeal;
- date of filing an appeal;
- signature of the applicant or his representative;
- other information provided for by legislation of the Republic of Kazakhstan.
If the administrative procedure is on a paid basis, the applicant must submit a document confirming the payment.
