Reception of documents for republican specialized boarding schools-colleges of the Olympic reserve and regional boarding schools for children gifted in sports

5. In personal account ("History of obtained services") familiarize with notification on your request processing status, which will be sent within the stated period.

4. Fill in a request and sign it using one-time password (SMS will be sent to a phone number indicated in a personal account, registration in citizens` mobile database is required)

Documents required for submission to service provider:

1. Acceptance of documents for admission to grades 5, 6, 7, 8, 9 of specialized Olympic Reserve schools/boarding schools/colleges (hereinafter: SORSBSC) and boarding schools for children gifted in sports (hereinafter: BSCGS).
2. Acceptance of documents for admission to grades 10 and 11 of SORSBSC and BSCGS:

1. An application in any form from one of the parents (legal representative).
2. A document (birth certificate with an individual identification number) or a digital document from the Digital Documents service certifying an applicant`s identity (for identification).
3. A document or a digital document from the Digital Documents service certifying an identity of a parent or legal representative (for identification).
4. An athlete's personal card filled-in according to the established format.
5. A certificate of basic secondary school completion or a certificate of general secondary education (for admission to grades 10 and 11) or a digital document from the Digital Documents service.
6. A copy of certificate or order or an extract from the order on assignment of the following sports titles or sports categories: "Honoured Master of Sports of the Republic of Kazakhstan", "Master of Sports of International Class of the Republic of Kazakhstan", "Master of Sports of the Republic of Kazakhstan", "Candidate Master of Sports of the Republic of Kazakhstan", first-class sportsman, second-class sportsman, third-class sportsman, first-class junior sportsman, second-class junior sportsman, third-class junior sportsman, when applying for 5, 6 and 7 grades, an abstract from a sports organization on completion of initial training;
7. Six photos (3x4).
8. An original copies of medical records kept in outpatient organizations pursuant to the Appendix 3 to the Order of the Acting Minister of Health of the Republic of Kazakhstan as of October 30, 2020, No. KR DSM-175/2020 "On approval of accounting documentation in healthcare" (registered in the Register of state registration of regulatory legal acts under No. 21579) (hereinafter: Order of the Acting Minister of Health of the Republic of Kazakhstan as of October 30, 2020, No. KR DSM-175/2020), namely, medical certificate in form No. 075/y (medical professional statement) with data on complete blood count and urine analysis issued not earlier than 10 calendar days prior to the date of application submission, statements issued by dentist, endocrinologist, cardiologist, phthisiatrician (in case of constant positive Mantoux test), outpatient medical card in form No. 052/y (if available), vaccination record card in the form No. 065/y, as well as a medical certificate of the epidemiological environment, medical statement on ultrasound results of heart, kidneys, abdominal organs, medical statement on electroencephalogram, and magnetic resonance imaging of the brain (in case of applying to box).
9. Сopies of diplomas, certificates, competition reports certifying obtainment of prizes at regional competitions, competitions in cities of republican significance, the capital that are included into the unified regional calendar of sports events (if available)for those applying to the republican SORSBSC.
10. Сopies of diplomas, certificates, competition reports certyfing obtainment of prizes at regional competitions, competitions in cities of republican significance, the capital that are included into the unified regional calendar of sports events (if available): for those applying to regional SORSBSC and BSCGS.

3. Acceptance of documents for admission to the first year of college in SORSBSC:

1. An application in any form.
2. A document or a digital document from the Digital Documents service certifying an applicant`s identity (for identification).
3. A certificate of general secondary education or a digital document from the Digital Documents service.
4. A Unified National Testing certificate or a comprehensive testing certificate (if available).
5. A copy of certificate or order (extract from an order) on assignment of the following sports titles or sports categories: "Honoured Master of Sports of the Republic of Kazakhstan", "Master of Sports of International Class of the Republic of Kazakhstan", "Master of Sports of the Republic of Kazakhstan", "Candidate Master of Sports of the Republic of Kazakhstan".

1. Establishing the unreliability of the documents submitted by the service recipient for the public service receipt and (or) the data (information) contained therein.

2. Non-compliance of the service recipient and (or) the data and information provided, necessary for the provision of public services, with the requirements established by this List of basic requirements for the public services provision and paragraph 5 of these Rules.

3. Absence of the consent of the service recipient provided in accordance with Article 8 of the Law of the Republic of Kazakhstan "On Personal Data and Their Protection", for access to personal data of limited access, which are required for the public services provision.

 General requirements for the appeal

The appeal submitted in hard copy or electronic form shall indicate:

• Full name, IIN, postal address of a natural person, BIN of a legal entity;
• name of administrative body, official to whom the appeal is submitted;
• the essence of an appeal;
• date of filing an appeal;
• signature of the applicant or his representative;
• other information provided for by legislation of the Republic of Kazakhstan.

If the administrative procedure is on a paid basis, the applicant must submit a document confirming the payment.
