Receipt of documents and enrollment of children to pre-school educational institutions

You can obtain the service via the Portal:

Using the EDS:

1.Log in to the portal or register

2. Click on the "Request online" button

3. Fill all the e-enquiry sections

4. Sign the request with digital signature 

5. In the personal account ("History of obtained services") be familiar with your request processing status notification which will be sent within the stated period.

Using one-time password:

1.Log in to the portal or register

2. Click on the "Request online" button

3.  Fill all the e-request sections

4. Fill in the request and sign it using one-time password (an SMS will be sent to the phone number indicated in the personal account, registration in the citizens` mobile database is required)   

5. In the personal account ("History of obtained services") be familiar with the application processing status notification which will be sent within the indicated period.

Documents required for submission to the service provider:

1. An application.
2. Preventive vaccination card form No. 065/у, approved by order of Acting Minister of Health of the Republic of Kazakhstan dated October 30, 2020 No. ҚР ДСМ -175/2020 “On approval of health accounting documentation forms as well as instructions for filling them out” (registered in the State registration Register of statutory legal acts under No. 21579).
3. Certificate of form No. 052-2/у “Child’s Health Passport”, approved by Order No. ҚР ДСМ -175/2020.
4. Certificate of form No. 027/у, approved by Order No. ҚР ДСМ -175/2020.
5. An e- copy of psychological, medical and pedagogical consultation conclusion (for children with disabilities) (if available).
6. An e-copy of a certificate issued from the place of work of a military personnel or employee of special government agencies, law enforcement agencies, certified by the signature of an authorized person and seal (valid for a month from the date of issue).
7. Certificate from the place of work of a teacher or medical worker certified by the organization’s head signature and seal (valid for a month from the date of issue).

The information about identity documents, child’s birth certificate, as well as the information confirming the priority receipt of referral to a preschool educational institution is pulled out from the relevant state information systems.

For non-residents:

1. An application.
2. An e-copy of identity document of the service recipient (one of the parents or legal representative).
3. An e-copy of the child’s birth certificate.
4. Preventive vaccination card form No. 065/e, approved by Order No. ҚР ДСМ -175/2020.
5. Certificate of form No. 052-2/у “Child Health Passport”, approved by Order No. ҚР ДСМ -175/2020.
6. Certificate of form No. 027/e, approved by Order No. ҚР ДСМ -175/2020.
7. An e-copy of psychological, medical and pedagogical consultation conclusion (for children with disabilities) (if available).

1. Establishing the unreliability of the documents submitted by the service recipient to receive the public service, and (or) data (information) contained therein.
2. The non-compliance of the service recipient and (or) presented materials, objects, data and information required for the public services provision with the List of requirements established by these rules.
3. The absence of consent of the service recipient, provided in accordance with Article 8 of the Law of the Republic of Kazakhstan “On Personal Data and Their Protection,” to access restricted personal data that is required for the public service provision.

 General requirements for the appeal

The appeal submitted in hard copy or electronic form shall indicate:

• Full name, IIN, postal address of a natural person, BIN of a legal entity;
• name of administrative body, official to whom the appeal is submitted;
• the essence of an appeal;
• date of filing an appeal;
• signature of the applicant or his representative;
• other information provided for by legislation of the Republic of Kazakhstan.

If the administrative procedure is on a paid basis, the applicant must submit a document confirming the payment.
