Preparation of documents for the provision of special social services in medical and social institutions (organizations)

 On the portal you can get the service in several ways:

Using EDS:

1. Log in to the portal or register

2. Click on "Request online" button

3. Fill all sections of e-request

4. Sign a request with digital signature 

5. In personal account ("History of obtained services") familiarize with notification on your request processing status, which will be sent within the stated period.

Using one-time password:Using one-time password:

1.Log in to the portal or register

2. Click on "Request online" button

3.  Fill all sections of e-request

4. Fill in a request and sign it using one-time password (SMS will be sent to a phone number indicated in a personal account, registration in citizens` mobile database is required)   

5. In personal account ("History of obtained services") familiarize with notification on application processing, which will be sent within the indicated period.

Documents required for submission to "Government for Citizens" Public Corporation":

1. Application in the form pursuant to Appendices 1 and 2.

2. An ID document or an electronic document from Digital Documents service indicating an individual identification number (for identification).

3. Medical record in the forms pursuant to Appendices 5 and 6 to the Rules and abstract from outpatient medical record. 

Information about the identity document, on the establishment of disability (for the elderly in the presence of disability), on the measures developed in the individual program of habilitation and rehabilitation of a person with disabilities (hereinafter – IPR) (for the elderly in the presence of IPR), on the recognition of a person over the age of 18 with neuropsychiatric diseases as incapacitated by a court decision (if any), on retirement age, on the status of a veteran of the Great Patriotic War, a veteran of military operations on the territory of other states, as well as a veteran equated to veterans of the Great Patriotic War on benefits, families of the deceased servicemen listed in subparagraph 1) of Article 8 of the Law of the Republic of Kazakhstan "On Veterans" (for veterans of the Great Patriotic War, veterans of military operations on the territory of other states, as well as veterans equated by benefits to veterans of the Great Patriotic War, families of deceased servicemen listed in subparagraph 1) of Article 8 of the Law of the Republic of Kazakhstan "On Veterans"), the service provider or a State corporation receives in the form of electronic documents certified by an electronic digital signature of authorized officials from the relevant state information systems. In the absence of information from information systems, a copy of the court decision on the recognition of a person as legally incompetent (if any) is attached to the application for persons over the age of eighteen.
Service providers receive digital documents from the digital document service through implemented integration, subject to the consent of the document owner provided through the user's cellular subscriber number registered on the e-government web portal by transmitting a one-time password or by sending a short text message as a response to the notification of the e-government web portal.

Documents required for submission to service provider:

1. Application in the form pursuant to Appendices 1 and 2.

2. An ID document or an electronic document from Digital Documents service indicating an individual identification number (for identification).

1) establishing the unreliability of the documents submitted by the service recipient for receiving a public service, and (or) the data (information) contained therein;
2) submission by the service recipient of expired documents and (or) an incomplete package of documents in accordance with the list provided for in paragraph 8 of this list of basic requirements for the provision of public services;
3) non-compliance of the service recipient and (or) the submitted materials, objects, data and information contained therein necessary for the provision of public services, in accordance with the requirements established by the Rules of Activity of organizations providing special social services, approved by Order No. 379 of the Minister of Labor and Social Protection of the Republic of Kazakhstan dated August 29, 2018 (registered in the Register state registration of regulatory legal acts No. 17467);
4) the absence of the consent of the service recipient, provided in accordance with Article 8 of the Law of the Republic of Kazakhstan "On Personal Data and their protection", for access to restricted personal data required for the provision of public services.

• appeal;
• initiative of an administrative body or official.

 General requirements for the appeal

The appeal submitted in hard copy or electronic form shall indicate:

• Full name, IIN, postal address of a natural person, BIN of a legal entity;
• name of administrative body, official to whom the appeal is submitted;
• the essence of an appeal;
• date of filing an appeal;
• signature of the applicant or his representative;
• other information provided for by legislation of the Republic of Kazakhstan.

If the administrative procedure is on a paid basis, the applicant must submit a document confirming the payment.
