Request for documents from Kazakhstan

• Completed form;
• Three copies of the completed questionnaire (answers in the questionnaire must be given fully and accurately in Kazakh or Russian, or with Kazakh or Russian translation);
• A copy of the proof of identity of an applicant, a legal representative or a trustee;
• A copy of the document confirming the relationship of the applicant with the person in respect of whom the documents are requested (if necessary);
• An original document confirming the rights of a legal representative or a trustee (if documents are requested by a legal representative who has the right to receive them on your behalf);
• Consent of the applicant, legal representative or trustee to the collection and processing of personal data;
• A copy of the requested document (if available).

* Death certificates may be requested by relatives of the deceased or legal entities for official purposes;

* Requests for reclamation in respect of minors, as well as persons recognized by a court as legally incapable, shall be submitted by their legal representatives (parents, guardians, custodians).

• Up to 4 months

In case of necessity of additional verification by state bodies and organizations of the Republic of Kazakhstan, as well as sending documents by mail, which requires a long time, the period of reclamation shall be extended for no more than thirty calendar days, about which the applicant shall be informed.

• 30 USD (for each document)

• Rules for requesting documents related to ensuring the rights and legitimate interests of citizens of the Republic of Kazakhstan, foreign citizens and stateless persons through the institutions of the Ministry of Foreign Affairs of the Republic of Kazakhstan

Acceptance of documents: daily (except Wednesday, Friday and Saturday, as well as holidays) from 09.30 to 12.30 h.

Issuance of documents: from 14:00 to 15:00 hrs.
