Identity document.
When transporting products, raw materials of animal origin, feed (except for products marked with the symbol "*" in the Unified List of Goods Subject to Veterinary Control (Surveillance), approved by the Decision of the Commission of the Eurasian Economic Union dated June 18, 2010 No. 317) - a copy of examination report (test protocol).
It is allowed to use the examination report (test protocol) within 1 month from the date of its issue for transportation (export) of chilled meat, chilled food fish products, edible eggs from the production facility engaged in raising of animals, procurement (slaughter), storage, processing and sale of animals, products and raw materials of animal origin provided that:
- there is a Product Monitoring Study Plan approved by the production facility and agreed upon by territorial division of the authorized body in the field of veterinary medicine of the relevant administrative-territorial unit;
- the compliance of products examined within the framework of Monitoring Plan for the studied safety indicators during the period of use of examination report (test protocol) within 1 month from the date of its issue;
When moving (transporting) dogs and cats - a copy of veterinary passport;
When moving (transporting) objects moved (transported) from one administrative-territorial unit of the republic to another administrative-territorial unit of the republic and / or with the change of the owner of the moved (transported) object within one administrative-territorial unit of the republic, for the purpose of export to the member states of the Eurasian Economic Union or third countries (states that are not members of the Eurasian Economic Union) - a copy of veterinary certificate (in the absence of a single automated agro-industrial sectors management system "e-Agriculture";
A copy of document certifying the payment for a veterinary certificate specimen.

Veterinary certificate, or a motivated refusal to provide a public service.
1) the deterioration of epizootic situation at the place of origin (location), route and (or) arrival (destination) of the object of state veterinary and sanitary control and surveillance for especially dangerous and contagious animal diseases;
2) the confirmation of a threat to the safety of animal and human health based on the results of monitoring the safety of the object of state veterinary and sanitary control and surveillance;
3) the identification based on the results of state veterinary and sanitary control and surveillance, of the violation of the requirements of the legislation of the Republic of Kazakhstan in the field of veterinary medicine that posed a threat to the safety of animal and human health;
4) the establishment of the inaccuracy of the documents submitted by the service recipient to obtain a veterinary certificate, and (or) data (information) contained therein;
5) the non-compliance of the service recipient and (or) submitted documents, moved (transported) object, vehicle, data and information necessary for the public service provision with the requirements established by the Rules;
6) in relation to the service recipient, there is a court decision (sentence) that has entered into legal force prohibiting the activity or individual types of activity related to obtaining a veterinary certificate.
