Documents acceptance and enrollment to additional education institutions of children
Documents required for submission to "Government for Citizens" Public Corporation":
1. An application from one of the parents (or other legal representatives).
2. A document certifying the identity of a child or an electronic document from the Digital Documents service (for identification).
3. Medical certificate in the No. 027/u form approved by the Order of the Acting Minister of Health of the Republic of Kazakhstan as of October 30, 2020 No. KR DSM-175/2020 (registered in the Register of State Registration of Regulatory Legal Acts under No. 21579).
4. The consent of the recipient of the provided service to access limited personal data required for the provision of public services.
1) orphans, children left without parental care;
2) children with special educational needs, the disabled and lifelong disabled, disabled children;
3) children from large families;
4) children in juvenile adaptation centers and support centers for
children in difficult life situations;
5) children living in boarding schools of general and sanatorium types, boarding schools at schools;
6) children brought up and studying in specialized boarding
educational institutions for gifted children;
7) pupils of boarding organizations;
8) children from families eligible for state targeted social
assistance, as well as from families not receiving state targeted social
assistance, in which the average per capita income is below the subsistence minimum;
9) children who, for health reasons, have been studying
for a long time under the programs of primary, basic secondary, general secondary education
at home or in organizations providing inpatient care,
as well as restorative treatment and medical rehabilitation;
10) other categories of citizens determined by the laws of the Republic of Kazakhstan;
11) other categories of citizens determined by the decision of the Government of the Republic of Kazakhstan;
12) individuals that do not belong to any of the above categories.

Issuance of a receipt for acceptance of documents pursuant to the Appendix 2 to the standard and when enrolling to a further education organization: a copy of the enrollment order.

2) отсутствие согласия услугополучателя на доступ к персональным данным ограниченного доступа, которые требуются для оказания государственной услуги в соответствии Законом Республики Казахстан «О персональных данных и их защите».

General requirements for the appeal
The appeal submitted in hard copy or electronic form shall indicate:
- Full name, IIN, postal address of a natural person, BIN of a legal entity;
- name of administrative body, official to whom the appeal is submitted;
- the essence of an appeal;
- date of filing an appeal;
- signature of the applicant or his representative;
- other information provided for by legislation of the Republic of Kazakhstan.
If the administrative procedure is on a paid basis, the applicant must submit a document confirming the payment.
