There are three ways to attach to a polyclinic: 1. by contacting the selected medical organization directly; 2. through the egov portal.kz, 3. submit an application to the PSC. Via eGov.kz it's easier to do. You need to log in to the portal and find the name of the service in the "Healthcare" tab, select it and click the "Order service online" button.
