Special social services in the home environment are provided:disabled persons, pensioners who are unable to serve themselves and who need special social services in the conditions of home care for health reasons, who do not have able-bodied adult children, spouse (gu),who are obliged in accordance with the Code of the Republic of Kazakhstan of December 26, 2011 "On Marriage (Matrimony) and Family" to support their disabled parents, spouse (gu) in need of assistance and take care of them, or who have able-bodied adult children, spouse( gu), who, for objective reasons, cannot provide them with permanent assistance and care (have a disability of the first, second group, oncological, mental diseases, are in prison, are registered in a drug treatment clinic or have left for permanent residence outside the country or live in another locality, in addition to the spouse (gi) – being in old age).
To receive a public service, the following documents are required::
1) a document certifying the identity of the service recipient with the presence of an individual identification number(for identification of the individual);
2) medical card in the form according to appendix 2 to this standard of public services;
3) a copy of the extract from the individual rehabilitation program of the disabled person (for the elderly is not required);
4) for persons of retirement age – a copy of the pension certificate;
5) for participants and disabled persons of the Great Patriotic War and persons equated to them – copies of the certificate confirming the status of the participant and disabled person of the Great Patriotic War and the person equated to them;
6) for children – a copy of the conclusion of the psychological, medical and pedagogical consultation.
Acceptance of documents for the provision of the state service "Registration of documents for the provision of special social services in the conditions of home care" is carried out in the State Institution "Department of Employment and Social Programs of the city of Temirtau" (22 Tchaikovsky Street) or in the NAO " GC "Government for Citizens" (PSC),work schedule:from Monday to Saturday inclusive, in accordance with the work schedule from 9.00 to 20.00 hours without a lunch break, except on Sundays and public holidays, according to the Labor Code of the Republic of Kazakhstan.
The documents are submitted in originals and copies for verification, after which the original documents are returned to the service recipient.
