The State Institution "Department of Employment and Social Programs of the Abai district", in accordance with the rules for the provision of public services "Registration of citizens affected by nuclear tests at the Semipalatinsk nuclear Test site, payment of one-time state monetary compensation, issuance of certificates", accepts applications and issues the result of the provision of public services through:
1) the service provider;
2) non–profit joint stock Company "State Corporation "Government for Citizens" (hereinafter - the State Corporation).
The service recipient, when applying for the provision of public services, provides an application(s) in accordance with the established forms and the following documents:
an identity document (required for identification); documents confirming the fact and period of residence on the territory of the Semipalatinsk test site in the periods from 1949 to 1965, from 1966 to 1990 (archival certificates); work record; graduation diploma; military ID; birth certificate, certificate of basic secondary education or general secondary education, certificate of marriage (matrimony) (certificate of state registration of acts of civil status) or certificate of dissolution of marriage (matrimony) or an extract from the certificate of dissolution of marriage (matrimony) (if the surname is changed); if available – certificate, issued earlier.
If the documents confirming the fact and period of residence on the territory of the Semipalatinsk test site have not been preserved, then a court decision is submitted to establish the legal fact and period of residence on the territory exposed to nuclear tests.
