The state service "Registration of documents for the provision of special social services in the conditions of home care" is provided by:
1) disabled children with neuropsychiatric pathologies;
2) disabled children with disorders of the musculoskeletal system;
3) persons with disabilities over the age of eighteen with neuropsychiatric diseases;
4) disabled persons of the first and second groups;
5) persons who are not capable of self-service due to their advanced age.
Acceptance of the application and issuance of the result of the provision of public services are carried out through:
1) non-profit joint-stock company "State Corporation" Government for Citizens";
2) State Institution "Department of Employment and social programs of Abay district".
The service recipient (or his legal representative), when applying for the provision of public services, provides the following documents to the State Institution "Department of Employment and Social Programs of the Abay District", to the State Corporation:
1) application form;
2) a document certifying the identity of the service recipient with the presence of an individual identification number (for identification of the individual);
3) medical card in the form of;
4) for children – a copy of the conclusion of the psychological, medical and pedagogical consultation.
If the information of the service recipient is not available in the information systems, the following documents are attached to the application:
1) a copy of the statement from the IPR (for the elderly in the presence of IPR);
2) for participants and disabled persons of the Great Patriotic War and persons equated to them-a copy of the certificate confirming the status of a participant and disabled person of the Great Patriotic War and a person equated to them.
The documents are submitted in originals and copies for verification, after which the original documents are returned to the service recipient, except for the medical card.
