List of documents accepted from mothers with many children

1) a copy of the identity card;

2) a copy of the house book or address reference;

3) a copy of the certificate of assignment of the title "Mother Heroine" (in case of awarding);

4) copies of the marriage certificate;

5) copies of children's birth certificates;

6) address certificates or house books confirming the place of residence of children;

7) if the child was adopted, copies of documents confirming the adoption;

8) if the children died or went missing in the defense of the interests of the Republic of Kazakhstan or in performance of other official duties, was killed during natural disasters or when performing a civil duty to rescue human life and property, in combating crime and maintaining public order, and died as a result of wounds, injury or illness obtained under these circumstances or as a result of occupational injury or disease copies of documents confirming the death or disappearance of missing children.
