Applying for a wheelchair

Today we will tell you how to use the state service for providing wheelchairs for disabled people. This can be done in two ways:

 

Registration through the PSC:

1. Book your visit to the preferential window at the PSC and arrive 20 minutes before the appointed time;
2. Take the following documents with you:

- Identification;

- For participants and disabled people of the Second World War and persons equated to them - copies of a certificate confirming the status of a participant and disabled person of the Second World War and a person equated to them;

- Copy of an extract from the individual rehabilitation program;

- If a person is incapacitated, then an official representative or a representative by proxy applies to the PSC.

 

Home decoration:

Leave a request for registration of services at home through the Single Contact Center 1414. After submitting the application, the PSC operator will contact you for additional information.

 

Service term:

10 working days, excluding the day of submission of documents.

 

Service result:

As soon as an SMS message about readiness arrives, you need to pick up a ready-made solution for issuing a stroller.

 

Attention, a wheelchair based on the decision received at the PSC is issued by the Office of Employment and Social Programs.
