ADMINISTRATIVE PROCEDURAL AND PROCEDURAL CODE OF THE REPUBLIC OF KAZAKHSTAN

This Code comes into force on July 1, 2021.
Article 63. General requirements for applying
An appeal submitted orally is recorded in a separate protocol by an official or an employee of the administrative body who accepted the appeal.
In an appeal submitted in writing or in the form of an electronic document, the protocol shall specify:
1) surname, first name, patronymic (if it is indicated in the identity document), individual identification number (if any), postal address of an individual or name, postal address, business identification number of a legal entity (if any);
2) the name of the administrative body, the official to whom the appeal is submitted;
3) the essence of the appeal;
4) date of submission of the application;
5) signature of the applicant or his representative;
6) other information provided by the legislation of the Republic of Kazakhstan.
