- application in the form of an electronic document;
- an electronic copy of the document confirming the payment of the license fee to the budget;
- an electronic copy of the lease or gratuitous use agreement for stationary premises used for purposes other than permanent residence, located outside buildings and on the territories of health care organizations, education, sports and recreation, sports and technical facilities, gas stations, shopping markets, cultural and leisure organizations on the right of ownership or on the right of temporary possession (use), gratuitous use – in the case of the conclusion of these contracts for a period of less than one year;
