Provision of wheelchairs for disabled people

Via PSC:
Queue at the PSC;

Here are the documents you need:
ID card;
Attorney's power of attorney (if notarization is not required));
Information about the medical rehabilitation program;
Certificate of disability;
If the employee is a participant in the second world war or a person equated to it, a certificate of this;
If the case concerns an incapacitated citizen, then his legal representative can submit documents to the PSC.
File maintenance time:
within 10 business days, excluding the date of submission of documents.
Result of the service:
when you receive an SMS message to your mobile phone, you can get a decision on providing a wheelchair at the PSC.
