To do this, it is enough to have an identity card, and for privileged categories of citizens, a document confirming the status (pension certificate, guardianship documents, birth certificate, etc.).
There are three ways to attach to a polyclinic:
1. by contacting the selected organization directly;
2. through the egov portal.kz,
3. submit an application to the PSC.
Via eGov.kz it's easier to do.
You need to log in to the portal and find the name of the service in the "Healthcare" tab, select it and click the "Order service online" button. The completed application must be signed by an EDS.
The result will be sent to your personal account.
You can attach to the polyclinic from September 15 to November 15.
