Procedure for state registration of changes first name, middle name, last name

State registration of a change of name, patronymic, surname is made by the registration authorities on the personal application of a person who has reached the age of sixteen and wants to change the name and (or) patronymic, surname.
Change of the name, patronymic, surname is made for the following valid reasons:
1) dissonance of the first name, patronymic, surname;
2) difficulty pronouncing first name, patronymic, last name;
3) the desire of the spouse to share the same surname with the spouse, if at the state registration of marriage (matrimony) remained on premarital surnames;
4) the desire to bear a premarital surname, if this is not stated at the dissolution of the marriage (matrimony);
5) the desire to bear a surname shared with children from a previous marriage (matrimony);
6) the desire to bear a pre-marital surname in the event of the death of the spouse;
7) the desire to share a surname with the children in the event of the death of the spouse, and if the applicant was on a pre-marital surname;
8) the desire to bear the name and (or) surname corresponding to the nationality of one of the parents chosen by the applicant (for different nationalities of the parents);
9) the desire to bear a name that is actually formed in life, different from the name in documents;
10) the desire to bear a premarital surname, if at the time of marriage (matrimony) the surname of the spouse was adopted;
11) the desire to bear the name of the father or grandfather according to national traditions;
12) the desire to bear the surname and (or) patronymic of the person who actually raised the applicant;
13) the desire to wear a first name, patronymic, surname corresponding to the chosen gender, in case of surgical gender change.
The application for the change of name, patronymic, last name is the registering authority in the manner prescribed by the legislation of the Republic of Kazakhstan on order of consideration of addresses of physical and legal entities.
An application to change the name, patronymic, surname is submitted to any registration authority in the territory of the Republic of Kazakhstan at the choice of the applicant.If the application for a change of name, patronymic, or surname is granted, state registration is made at the place where the application is submitted.
The following documents are attached to the application for changing the name, patronymic, surname, confirming the reasons for which the applicant requests to change the name, patronymic, surname:
1) the applicant's birth certificate;
2) certificate of marriage (matrimony), if the applicant is married (matrimony);
3) a child's birth certificate, if the applicant has minor children;
4) certificate of adoption or certificate of establishment of paternity, if such were registered by the registration authorities;
5) certificate of dissolution of marriage (matrimony), if the applicant applies for the assignment of a premarital surname in connection with the dissolution of marriage (matrimony);
6) two photos of the applicant.
If necessary, additional documents confirming the reasons for which the applicant requests to change the name, patronymic, and surname are requested.
REJECTION. The refusal of the registering authority to change the name, patronymic, surname may be appealed in court.
In connection with the change of name, patronymic, surname on the basis of the act record of this change is made only in the act record of the birth of the person in respect of which it is made. No changes are made to other entries.
In confirmation of the made change of the name, patronymic, surname the certificate on change of the name, patronymic, surname of the established sample is shown.
After the state registration of the change of name, patronymic, surname, a citizen is issued a certificate of change of name, patronymic, surname.
A certificate of change of name, patronymic, surname is issued at the place of state registration only to the person in respect of whom the change was made.
