State registration of full name change abroad

Changes name, surname, father`s name is made on the basis of personal application (download) of those who has reached the age of 16. In a statement about the change of name, surname, father`s name following information must be indicated:

• Name, surname, father`s name;
• Intended name, surname, father`s name;
• Reasons for change name, surname, father`s.

To application following documents should be attached:

1. Birth certificate of applicant;
2. Marriage certificate (if applicant is married);
3. Birth certificate(s) of child (if applicant have underage children);
4. Certificate of adoption of child (in case of adoption);
5. Certificate of Divorce (if applicant wishes to change surname, name and father`s name before marriage);
6. Two photos.

In case of need an additional documents could be requested.
