- Three copies of the questionnaire (answers must be complete and accurate, provided in Kazakh or Russian).
- Copy of the applicant’s identity document, legal representative, or authorized person’s ID.
- Copy of the document confirming the familial relationship between the applicant and the person whose documents are being requested (if applicable).
- Original document confirming the authority of the legal representative or authorized person (if the documents are requested by such a representative).
- Consent of the applicant, legal representative, or authorized person for the collection and processing of personal data (in free form).
- Copy of the requested document (if available).
- Original receipt confirming payment of the consular fee.
* Death certificates may be requested by relatives of the deceased or legal entities for official purposes;
* Applications for requests concerning minors or individuals declared legally incapable by court must be submitted by their legal representatives (parents, guardians, trustees).
