• Application (in free form).
• Three copies of the application form (all questions must be answered fully and accurately in Kazakh or Russian).
• A copy of the applicant’s identity document, or that of the legal representative or authorized person.
• A copy of a document confirming the applicant’s family relationship with the person whose documents are being requested (if applicable).
• The original document confirming the authority of the legal representative or authorized person (if the documents are requested by a representative of the entitled person).
• Consent of the applicant or his/her legal representative or authorized person for the collection and processing of personal data (in free form).
• A copy of the requested document (if available).
• The original document confirming payment of the consular fee. • Death certificates may be requested by relatives of the deceased or by legal entities for official purposes.
• Requests concerning minors, as well as persons declared legally incapable by a court, must be submitted by their legal representatives (parents, guardians, or custodians).
