- Original document confirming the authority of the legal representative or authorized person (if documents are requested by a representative authorized to receive them).
- Consent of the applicant, legal representative, or authorized person to the collection and processing of personal data (in free form).
- Copy of the requested document (if available).
- Original document confirming payment of the consular fee.
* Death certificates may be requested by relatives of the deceased or legal entities for official purposes;
* Requests for documents concerning minors and persons legally declared incapacitated must be submitted by their legal representatives (parents, guardians, trustees).
