You can apply for "B5" Crewmember Visa category (single entry until 30 days within 90 days/ multiple entry until 30 days at each entry within 1 year) if you are:
Passport (with at least two blank pages; it should be valid for at least three months longer than the validity of the requested visa).
One recent, colour passport sized (3.5cm x 4.5cm) picture of the applicant which should be attached to the marked space in the upper right-hand corner of the application form.

The Crewmember visa process takes about 5 business days from the moment the Consular Section receives the documents. Crewmember visa applications must be submitted in-person or using a third party accredited agencies. Applications are not accepted by mail.
