About the starting of document submission for first grade enrollment

Starting from April 1, 2024, acceptance of children’s documents for first grade enrollment in educational organizations of our region is underway.  

Parents can receive this service via the e-government portal www.egov.kz using an electronic digital signature (EDS). Applications are submitted 24/7, so parents can apply online without leaving their homes.

To submission an application, you need to visit the website www.egov.kz, fill out the application for parents or other legal representatives and attach the following scanned documents (PDF):

1) medical certificate of Form No. 052-2/u “Child's Health Passport”, approved by the Order (hereinafter referred to as the Order) of the Acting Minister of Healthcare of the Republic of Kazakhstan dated October 30, 2020 KR DSM–175/2020 “On approval of reporting documentation forms in healthcare, and instructions for their completion” (registered in the Register of State Registration of Regulatory Legal Acts No. 21579). It is worth noting that on the basis of the Order, Form No. 065/u “Preventive vaccination card” is already included in Form No. 052-2/u, thus it is enough to attach Form No. 052-2/u;

2) a 3x4 cm digital photo of the child

No computer or Internet? In this case, parents can visit the nearest school, where the staff of the education organization will help applicants with submitting documents on the e-government web portal.

It is also worth noting that to date, the Ministry of Healthcare of the Republic of Kazakhstan is currently working on converting Form No. 052-2/u into an electronic format. However, this process requires some time.

In this regard, Emergency Medical Service Organizations have been instructed to provide Forms No. 052-2/u in paper form at the request of parents with the seal of the medical organization, as well as the physician’s seal and signature.
