"Issuing a certificate to a rehabilitated person"
The state Institution "Department of Employment and Social Programs of the city of Sarani" provides the state service "Issuance of a certificate to a rehabilitated person".
The service is provided to rehabilitated persons who have been subjected to political repression (hereinafter referred to as the service recipient).
The acceptance of the application and the issuance of the result of the provision of public services are carried out through the non–profit joint stock company "State Corporation "Government for Citizens" (hereinafter - the State Corporation).
In order to receive a public service, the service recipient (or his representative under a power of attorney notarized by a notary or an official performing notarial actions) applies to the State Corporation by submitting an application with the attachment of documents necessary for the provision of public services.
Documents required for the provision of public services:
1) identity document (required for identification);
2) a certificate of rehabilitation or a copy of the ruling (resolution) of the court.
The documents are submitted in originals and for verification, after which the documents are returned to the applicant.
When submitting a full package of documents, the service recipient is issued a tear-off coupon indicating the date of registration, surname and initials of the person who accepted the documents.
The total period for reviewing documents and issuing the results of the provision of public services from the date of receipt and registration of the package of documents in the State Corporation is five working days.
The result of rendering a public service is a certificate or its duplicate in the form approved by the Decree of the Government of the Republic of Kazakhstan dated April 2, 2015 No. 184 "On Approval of the Rules for Issuing a single-sample certificate to a rehabilitated Person, a sample of a rehabilitated Person's certificate and Invalidation of the Resolution of the Cabinet of Ministers of the Republic of Kazakhstan dated October 22, 1993 No. 1055 " On the Procedure for providing certificates
rehabilitated persons who have been subjected to political repression."
The result of the provision of a public service is issued upon presentation of an identity document during a personal visit to the service recipient (or
his representative under a notarized power of attorney) on the basis of a detachable
the application form.
Grounds for refusal to provide public services:
1) establishing the unreliability of the documents submitted by the service recipient for receiving the state service, and (or) the data (information) contained therein;
2) inconsistency of the service recipient and (or) the submitted materials, data and information necessary for the provision of public services.
The complaint of the service recipient received by the service provider, akimat, is subject to consideration within 5 (five) working days from the date of its registration.
The complaint of the service recipient received by the authorized body for the assessment and control of the quality of public services is subject to consideration within 15 (fifteen) working days from the date of its registration.
