Standard of the state service
" Registration of changes in the name, patronymic, surname, including making changes, additions and corrections to the civil status records"

Name of the service provider
MIO of the cities of Nur-Sultan, Almaty and Shymkent, districts and cities of regional significance (hereinafter-the service provider)

Methods of providing public services (access channels)
1) through the service provider;
2) MIO of districts in the city, cities of district significance, akims of settlements, villages, rural districts;
3) State Corporation;
4) through the portal

Term of public service provision
to the service provider or to the State corporation - 5 (five) business days;
making changes, additions and corrections to the record of the civil status act - 5 (five) working days;
in the absence of an official record in the Civil Registry Office, the term of rendering the service is extended for no more than 15 (fifteen) calendar days, with the notification of the service recipient within 3 (three) calendar days (if the term of consideration of the application received through the State Corporation is extended, the result of rendering the state service is sent by the service provider to the State Corporation);
when submitting via the portal-a notification confirming the acceptance of the application and setting the date of registration of the change of name, patronymic, surname within 1 (one) business day is sent to the personal account;
The maximum allowed waiting time for the delivery of a package of documents is 20 (twenty) minutes.
The maximum allowed service time of the service recipient is 20 (twenty) minutes.

The result of the provision of public services
1) A certificate of change of name, patronymic, surname, birth certificate (in cases of amendments to the birth certificate), a repeated certificate of change of name, patronymic, surname with amendments, additions and corrections;
2) a reasoned response about the refusal to provide a public service.
The form of providing the result of the provision of public services: paper, electronic when a certificate of change of name, patronymic, surname or a reasoned refusal is sent when submitting an application through the portal.

The amount of payment charged to the service recipient for the provision of public services, and the methods of its collection in cases provided for by the legislation of the Republic of Kazakhstan
1) for registering a change of name, patronymic, surname – 2 MCI;
2) for issuing a certificate in connection with the change, addition, correction and restoration of the record of the act of changing the name, patronymic, surname – 0.5 MCI.
They are exempt from paying the state fee for the registration of civil status acts upon presentation of supporting documents:
1) participants and invalids of the Great Patriotic War and persons equated to them on benefits and guarantees, persons awarded orders and medals of the former USSR for selfless work and impeccable military service in the rear during the Great Patriotic War, persons who worked (served) at least six months from June 22, 1941 to May 9, 1945 and not awarded orders and medals of the former USSR for selfless work and impeccable military service in the rear during the Great Patriotic War, disabled people, as well as one of the parents of a disabled child from childhood, a disabled child, guardians( trustees), state organizations - for the registration and issuance of repeated birth certificates;
2) individuals - for issuing certificates to them when changing, supplementing, restoring and correcting civil status records in connection with errors made during the registration of civil status acts;

Work schedule
1) service provider - from Monday to Friday inclusive from 09.00 to 17.30 hours, break from 13.00 to 14.30 hours, except weekends and holidays.
The state service is provided on a first-come, first-served basis, without prior appointment and accelerated service;
2) State Corporation - from Monday to Saturday inclusive, in accordance with the established work schedule from 9-00 to 20-00 hours without a lunch break, except for Sundays and public holidays, according to labor legislation.
Reception is carried out in the order of "electronic" queue, without accelerated service, it is possible to book an electronic queue through the portal;
3) the portal – around the clock, with the exception of technical breaks associated with the repair work (when the service recipient applies after the end of working hours, on weekends and holidays, the application is accepted and the result of the provision of public services is issued on the next working day).

List of documents required for the provision of public services
1) application for state registration of a change of name, patronymic, surname in the form according to Appendix 22 to the Rules;
2) an identity document (for identification purposes);
3) certificates of civil status registration issued outside the Republic of Kazakhstan;
4) two photos of the service recipient in the size of 3*4 cm.;
5) a document confirming the payment of the state fee to the budget, except in cases of payment through the PSHEP;
6) if necessary, the employee of the service provider requests additional documents confirming the reasons for which the service recipient requests to change the name, patronymic, surname.
When accessing the portal:
1) an electronic application, certified by the EDS of the service recipient or certified by a one-time password, in the case of registration and connection of the subscriber number of the service recipient provided by the mobile operator to the portal account;
2) information on the state registration of civil status acts.
The list of necessary documents for making changes, additions and corrections to the act record of changing the name, patronymic, surname:
1) application for amendments, additions and corrections in the form according to Annex 24 to the Rules;
2) an identity document (for identification purposes);
3) the original of the certificate of state registration of the act of civil status, which is subject to exchange in connection with the introduction of changes, additions and corrections to the record of the act of civil status, in case of loss of the original certificate, indicate its loss and information about the registration of the act of civil status;
4) a document confirming the existence of grounds for making changes, additions and corrections to the record of the civil status act;
5) a notarized power of attorney, if the representative of the service recipient applies.

Grounds for refusal to provide public services established by the legislation of the Republic of Kazakhstan
1) establishing the unreliability of the documents submitted by the service recipient for receiving the state service, and (or) the data (information) contained therein;
2) non-compliance of the service recipient and (or) the submitted materials, objects, data and information necessary for the provision of public services with the requirements established by the regulatory legal acts of the Republic of Kazakhstan.
The laws of the Republic of Kazakhstan may establish other grounds for refusal to provide public services.

Other requirements, taking into account the specifics of the provision of public services, including those provided in electronic form and through a State corporation
Service recipients who, in accordance with the procedure established by law, have a complete or partial loss of the ability or ability to perform self-service, independently move, navigate, receive documents for the provision of public services by an employee of a State Corporation with departure at the place of residence by applying through a Single contact Center 1414, 8 800 080 777.
The service recipient has the opportunity to receive public services in electronic form through the portal, provided that the EDS is available.
The service recipient has the opportunity to obtain information about the procedure and status of the provision of public services in the remote access mode through the" personal account " of the portal, the reference services of the service provider, as well as the Single Contact Center "1414", 8-800-080-7777.
