The autobiography is executed without blemishes and corrections in chronological order by the person personally checked in purple or black ink with the mandatory indication of the following information:
Your last name, first name, patronymic (if any), date (date, month, year) and place (village, district, city, region) of birth, citizenship;
localities where you lived, indicating the exact addresses and periods of residence;
when, where and in which specialized secondary, technical secondary, higher educational institutions did you study, the numbers (name) of groups, faculties, the received specialty in education. If you have a certificate of education issued by foreign educational institutions, please provide information about the recognition or nostrification of your diploma;
when and by what department (department, department) for defense affairs you were called up for military service; specify information about service in educational institutions; service in the Armed Forces, other troops and military formations of the Republic of Kazakhstan or other countries of the Commonwealth of Independent States; type of troops; military specialty; position; places of deployment and numbers of military units; date of discharge to the reserve; registration with the military enlistment office, its name;
labor activity: specify the location, address and exact legal name of all enterprises and state institutions where you worked, the time of work, the position held, the reason for dismissal or transfer;
marital status: when they got married; last name, first name, patronymic (if any), year of birth, place of birth, nationality, citizenship of the spouse (last name is indicated before and after marriage); if they got married a second time, specify the details of the former spouse (s), the number and date of the divorce certificate, where, when, by whom it was issued;
if you and your spouse or close relatives (father, mother, full brothers, sister and children) have changed your surname, first name, patronymic (if any), specify their former surname, first name, patronymic (if any);
have you and your spouse or close relatives (father, mother, full brothers, sister and children) left the Republic of Kazakhstan (where, when, for what purpose);
do you or your spouse, close relatives living outside the Republic of Kazakhstan, or who previously lived abroad, including in the countries of the Commonwealth of Independent States (who, in which country, the degree of kinship);
was any of your close relatives or spouse in foreign citizenship or citizenship (surname, first name, patronymic (if any), degree of kinship); when and with whom of the above persons did you or your spouse or their relatives maintain contacts (written or other communications, including via the global Internet);
do you or your spouse, as well as close relatives (father, mother, full brothers, sister and children) have a criminal record;
have criminal cases been initiated against you or your spouse, as well as close relatives (father, mother, full-siblings, children), which were subsequently terminated by the criminal prosecution body or the court, or refused to initiate a criminal case by the criminal prosecution body on non-rehabilitating grounds;
have you been held accountable for committing administrative or corruption offenses;
have you been dismissed from the civil service for negative reasons;
Your current place of work and position, place of actual residence and residence permit (if available).
