Service Provider Name

The state service is provided by sports schools for children and youth, sports schools for the disabled (hereinafter referred to as the service provider).

Methods of providing public services

Acceptance of documents and issuance of the result of the provision of public services are carried out through the office of the service provider.

3.

The term for the provision of public services

From the moment the documents are handed over to the service provider - within 30 (thirty) minutes.

Form of provision of public services

Paper.

The result of the provision of public services

A copy of the application with a mark of acceptance indicating the date and surname of an employee of the service provider's office, a reasoned response to refuse to provide public services.

The amount of fees charged from the service recipient for the provision of public services, and the methods of its collection in cases provided for by the legislation of the Republic of Kazakhstan

Free for individuals.

7.

Service provider's working hours

Service Provider: Monday through Friday, from 09.00 to 18.30 hours with a lunch break from 13.00 to 14.30, except weekends and holidays, in accordance with the labor legislation of the Republic of Kazakhstan.
When the service recipient applies after the end of working hours, on weekends, holidays in accordance with the Labor Code of the Republic of Kazakhstan dated November 23, 2015 and the Law of the Republic of Kazakhstan dated December 13, 2001 "On Holidays in the Republic of Kazakhstan", the application is accepted and the result of the provision of public services is issued on next business day.
The addresses of the places of rendering public services are posted on the Internet resource of the Committee for Sports and Physical Culture of the Ministry of Culture and Sports of the Republic of Kazakhstan: www.gov.kz/memleket/entities/sport in the "Services" section.

List of documents required for the provision of public services

1) a statement from the parents or legal representatives of the applicant in any form;
2) the applicant's birth certificate with an individual identification number (for identification);
3) a document proving the identity of the parent or legal representative (for identification);
4) a medical certificate issued by a district doctor, indicating admission to classes in the chosen sport;
5) medical documentation in the form No. 088 / y, approved by order of the Acting Minister of Health of the Republic of Kazakhstan dated November 23, 2010 No. 907 "On approval of forms of primary medical documentation of health care organizations" (registered in the Register of state registration of normative legal acts under No. 6697) ( a medical and sanitary examination certificate at the place of residence is provided by people with disabilities).

Grounds for refusal to provide public services established by the laws of the Republic of Kazakhstan

1) establishing the inaccuracy of the documents submitted by the service recipient for receiving public services, and (or) the data (information) contained therein;
2) non-compliance of the service recipient and (or) the submitted data and information with the requirements provided for in clauses 9-1, 10-1 and 11 of these Rules.
