List of documents required for the provision of public services
|
Upon admission to grades 5, 6, 7, 8, 9, 10 and 11 of the republican specialized boarding schools-colleges of the Olympic reserve (hereinafter referred to as SSHIKOR) and regional boarding schools for gifted children in sports (hereinafter referred to as SHIOSD):
1) application from parents or legal representatives in any form;
2) an applicant's identity document or birth certificate (with an individual identification number);
3) an identity document of the parent or legal representative of the applicant;
4) an athlete's personal card filled out in the form in accordance with Appendix 3 to these Rules;
5) a certificate of completion of basic secondary school or a certificate of general secondary education (for admission to grades 10 and 11);
6) a copy of the certificate or order or an extract from the order on the assignment of the following sports titles or sports categories: "Honored Master of Sports of the Republic of Kazakhstan", "Master of Sports of International Class of the Republic of Kazakhstan", "Master of Sports of the Republic of Kazakhstan", "Candidate for Master of Sports of the Republic of Kazakhstan "," an athlete of the 1st category "," an athlete of the 2nd category "," an athlete of the 3rd category "," an athlete of the 1st youth category "," an athlete of the 2nd youth category "," an athlete of the 3rd youth category ".
A copy of the order or an extract from the order on the assignment of a sports rank is certified by the seal of the authorized body, and a copy of the order or an extract from the order on the assignment of a sports rank is certified by the seal of the local executive body;
a card of preventive vaccinations in the form No. 063 / y, as well as a medical certificate of the epidemiological environment; medical reports on ultrasound examination of the heart, kidneys, abdominal organs; a medical report based on an electroencephalogram, as well as a magnetic resonance imaging of the brain (in case of admission to the boxing department), a report from a phthisiatrician;
9) copies of diplomas, certificates, protocols of the competitions, testifying to the winning place at international or republican, or regional sports competitions included in the unified republican calendar of mass sports events (if any), for applicants to the republican SSHIKOR;
10) copies of diplomas, certificates, protocols of the competitions, testifying to the won prizes in the regional, cities of republican significance, the capital or city or district sports competitions included in the single regional calendar of mass sports events (if any) - for those entering the regional SSHIKOR and SHIOSD.
Upon admission to the first year of college at SSHIKOR:
1) application in any form;
2) the identity document of the applicant;
3) a certificate of general secondary education;
4) certificate of Unified national testing or comprehensive testing certificate (if any);
5) a copy of the certificate or order (extract from the order) on the assignment of the following sports titles and sports categories: "Honored Master of Sports of the Republic of Kazakhstan", "Master of Sports of International Class of the Republic of Kazakhstan", "Master of Sports of the Republic of Kazakhstan", "Candidate for Master of Sports of the Republic Kazakhstan".

6) six photos of 3x4 size;
7) the originals of medical records used in outpatient clinics, approved by Appendix 3 to the order of the acting Of the Minister of Health of the Republic of Kazakhstan dated November 23, 2010 No. 907, namely, a medical certificate in the form No. 086 / y (indicating data on the general analysis of blood, urine, issued no earlier than 10 calendar days before the date of application, conclusion of a dentist, endocrinologist, cardiologist , a phthisiatrician (in case of a constant positive Mantoux reaction), a child's medical record in form No. 026, a preventive vaccination card in form No. 063 / y, as well as a medical certificate of the epidemiological environment; medical reports on ultrasound examination of the heart, kidneys, abdominal organs ; a medical report based on an electroencephalogram,
8) copies of diplomas, certificates, protocols of competitions, testifying to the won prizes at international or republican sports competitions included in the single republican calendar of mass sports events (if any), for applicants to SSHIKOR;
9) an athlete's personal card, filled out in the form in accordance with Appendix 3 to the Rules.

Other requirements, taking into account the specifics of the provision of public services, including those provided in electronic form and through the State Corporation

The documents specified in clause 8 of this public service standard are accepted personally from parents or legal representatives of those entering grades 5, 6, 7, 8 and 9 from June 1 to August 20 of the current calendar year.
The documents specified in clause 8 of this standard of public services entering grades 10 and 11 are accepted from June 15 to August 20 of the current calendar year.
The documents specified in clause 8 of this public service standard are accepted personally from an applicant to college from June 20 to August 20 of the current calendar year.
