2) an identity document or an electronic document from the digital document service (for identification); 3) personal personnel and photo accounting sheet; 4) a copy of the state-issued document on education; 5) a copy of the document confirming employment; 6) certificate of health status in form075/y approved by Order of the Acting Minister of Health of the Republic of Kazakhstan dated October 30, 2020 No KR DSM175/2020 "On approval of forms of accounting documentation in the field of healthcare" (registered in the Register of State Registration of Regulatory Legal Acts under No. 21579); and (or) the presence of the first or highest qualification category of a teacher or the qualification of an expert teacher or a researcher teacher or a master teacher or the presence of the qualification category "deputy head of the third qualification category" or "deputy head of the second qualification category" or "deputy head of the first qualification category" of an educational organization or "headorganizer" or "headmanager" or "headleader"; work experience of at least five years as a civil servant in the field of education and science as the head of organizations of technical and vocational post-secondary or higher and (or) postgraduate education with the presence of pedagogical education, except for the norms of paragraph 3 of this paragraph;
