In accordance with subparagraph 1) of article 10 of the Law of the Republic of Kazakhstan dated April 15, 2013 "On public services" and subparagraph 6) of paragraph 1 of article 7 of the Code of the Republic of Kazakhstan dated September 18, 2009 "On public health and the health care system" I ORDER:
1. To add to the order of the Minister of Health and Social Development of the Republic of Kazakhstan dated April 27, 2015 No. 272 "On approval of standards of public services in the field of health care" (registered in the Register of State Registration of Normative Legal Acts No. 11304, published in the information and legal system "Adilet »June 23, 2015) the following change and addition:
paragraph 1 shall be stated in the following edition:

1) the standard of the public service "Doctor's home call" in accordance with Appendix 1 to this order;
2) the standard of the public service "Appointment to a doctor" in accordance with Appendix 2 to this order;
3) the standard of the public service "Attachment to a medical organization providing primary health care" in accordance with Appendix 3 to this order;
4) the standard of the public service "Voluntary anonymous and mandatory confidential medical examination for HIV infection" in accordance with Appendix 4 to this order;
5) the standard of the public service "Issuance of a certificate from an anti-tuberculosis organization" in accordance with Appendix 2 to this Order in accordance with Appendix 5 to this Order;
6) standard of the public service "Issue of a certificate from a neuropsychiatric organization" in accordance with Appendix 6 to this order;
7) the standard of the state service "Issuance of a certificate from a narcological organization" in accordance with Appendix 7 to this order;
8) the standard of the state service "Issuance of an extract from the medical card of an inpatient" in accordance with Appendix 8 to this order;
9) the standard of the state service "Issuance of a certificate from a medical organization providing primary health care" in accordance with Appendix 9 to this order;
10) the standard of the public service "Issuance of a certificate of temporary disability from a medical organization providing primary health care" in accordance with Appendix 10 to this order; 11) the standard of the public service "Issuance of a certificate of temporary disability of a medical organization providing primary health care" in accordance with Appendix 11 to this order; 12) the standard of the public service "Registration of consent or withdrawal of consent for an intravital voluntary donation of tissues (tissue parts) and (or) organs (parts of organs) after death for the purpose of transplantation" in accordance with Appendix 12 to this order. ";
supplement with Appendix 12 in accordance with the Appendix to this Order. 2. The Department for Organization of Medical Aid of the Ministry of Health and Social Development of the Republic of Kazakhstan shall ensure:
1) state registration of this order with the Ministry of Justice of the Republic of Kazakhstan; 2) after state registration of this order with the Ministry of Justice of the Republic of Kazakhstan, sending a copy of it for official publication in periodicals and in the information and legal system of the Republic of Kazakhstan "adilet"; 3) posting this order on the Internet resource of the Ministry of Health and Social Development of the Republic of Kazakhstan;
4) within ten working days after the state registration of this order with the Ministry of Justice of the Republic of Kazakhstan, the submission to the Department of the Legal Service of the Ministry of Health and Social Development of the Republic of Kazakhstan information on the implementation of the measures provided for in subparagraphs 1), 2) and 3) of this paragraph. 3. Control over the execution of this order shall be entrusted to the Vice-Minister of Health and Social Development of the Republic of Kazakhstan Tsoi A.V. 4. This order comes into effect upon the expiration of ten calendar days after the day of its first official publication.

Public service standard "Registration of consent or revocation of consent for lifetime voluntary donation of tissues (parts of tissue) and (or) organs (parts of organs) after death for the purpose of transplantation "
1. General Provisions
1. State service "Registration of consent or withdrawal of consent for an intravital, voluntary donation of tissues (parts of tissue) and (or) organs (parts of organs) after death for the purpose of transplantation" (hereinafter - the state service).
2. The standard of public services was developed by the Ministry of Health and Social Development of the Republic of Kazakhstan (hereinafter referred to as the Ministry).
3. The public service is provided by medical organizations providing primary health care (hereinafter referred to as the service provider).
The application is accepted and the result of the provision of public services is issued through the service provider.
2. The procedure for the provision of public services
4. The term for the provision of public services:
1) from the moment of delivery of the package of documents to the service provider - 3 (three) working days;
2) the maximum allowable waiting time for the submission of a package of documents - no more than 30 (thirty) minutes;
3) the maximum allowable service time is no more than 30 (thirty) minutes.
5. Form of rendering public services: paper.
6. Result of rendering public services:
1) a certificate of registration of consent to an intravital voluntary donation of tissues (parts of tissue) and (or) organs (parts of organs) in the form in accordance with Appendix 1 to this standard or a certificate of refusal to register consent to an intravital voluntary donation of tissues (parts of tissue) and ( or) organs (parts of organs) in the form in accordance with Appendix 2 to this standard;
2) a certificate of registration of withdrawal of consent for an intravital voluntary donation of tissues (tissue parts) and (or) organs (parts of organs) in the form in accordance with Appendix 3 to this standard.
7. The state service is provided to individuals free of charge. 8. The service provider's work schedule is from Monday to Friday from 8.00 to 20.00 without a break, on Saturday from 9.00 to 14.00, except weekends and holidays in accordance with the legislation of the Republic of Kazakhstan. The public service is provided on a first come, first served basis, without prior appointment and expedited service. 9. The list of documents required for the provision of public services when the service recipient applies: a document proving the identity of the service recipient; application in the form, in accordance with Appendix 4 or 5 to this standard of public service.
3. Procedure for appealing decisions, actions (inaction) service provider and (or) his officials on the provision of public service
10. Decisions, actions (inaction) of the service provider and (or) its officials on the provision of public services are appealed by filing a complaint addressed to the head of the service provider or the Ministry at the address specified in paragraph 13 of this public service standard, or at: 010000, g . Astana, Orynbor street, 8, House of Ministries, entrance number 5.
Confirmation of acceptance of the complaint is its registration (stamp, incoming number and date) in the office of the service provider or the Ministry, indicating the name and initials of the person who accepted the complaint, the time frame and place for receiving a response to the complaint. After registration, the complaint is sent to the head of the service provider or the Ministry to determine the responsible executor and take appropriate measures.
The service recipient's complaint sent to the service provider is subject to consideration within five working days from the date of its registration. A reasoned response on the results of the consideration of the complaint is sent to the service recipient by mail or is issued on purpose at the service provider's office.
In case of disagreement with the results of the rendered public service, the service recipient shall file a complaint with the authorized body for assessment and control over the quality of the provision of public services.
The service recipient's complaint received by the authorized body for the assessment and control over the quality of the provision of public services is considered within fifteen working days from the date of its registration.
11. In case of disagreement with the results of the provision of public services, the service recipient has the right to go to court in the manner prescribed by law.
4. Other requirements, taking into account the specifics of the provision public service
12. The premises of the service provider provide conditions for servicing service recipients with disabilities (ramps and lifts).
13. The addresses of the places of rendering public services are posted on the Internet resource of the Ministry www.mzsr.gov.kz, section "Public services", as well as Internet resources of the Health Departments of the regions, as well as the cities of Astana and Almaty.
14. The service recipient has the right to receive information on the procedure and status of the provision of public services by the contact numbers of the service provider.
5. Contact numbers of inquiry services on the provision of public services are indicated on the Ministry's Internet resource www.mzsr.gov.kz. A single contact center for the provision of public services: 8-800-080-7777, 1414.
